A sleep apnea appliance-also known as Oral Appliance Therapy or a Mandibular Advancement Device-is a custom-made device worn during sleep. It looks similar to a nightguard and consists of two components that fit snugly over the upper and lower teeth.
These two parts are connected by a small adjustment mechanism, typically a screw. This mechanism gently shifts the lower jaw (mandible) forward. Advancing the jaw helps open the airway at the back of the throat and repositions the tongue, reducing airway collapse and improving nighttime breathing.
Why We Don't Recommend Over-the-Counter Devices
While over-the-counter (OTC) appliances may seem appealing due to their affordability, they come with significant risks. These devices are not custom-fitted to your teeth and are not designed by a trained professional. As a result, they may:
- Apply uneven pressure on your jaw joints
- Cause discomfort or pain
- Lead to jaw misalignment or worsening bite issues
- Fail to hold your airway open effectively
Perhaps most importantly, OTC devices lack professional supervision. Without proper monitoring, bite changes and dental side effects can go unnoticed and unmanaged.

In contrast, custom appliances are designed using precise dental impressions and are individually adjusted to fit your mouth and airway needs. Regular check-ins allow your provider to monitor your bite, ensure comfort, and make necessary refinements over time.
